Veterinarian Licensure and Relocation Processes

New York State Veterinary Licensure

Bronx Veterinary Center will cover all expenses for obtaining your New York State (NYS) veterinarian license. The process and related forms are located on the New York State Education Department website.

As a guide, please complete the following forms and submit them to the NYS Board.

Follow the instructions and submit applicable fees. 

  • Form 2 – Certification of Professional Education
    1. Select Form 2, Certification of Professional Education. Complete Section 1 of the form and follow the instructions for Section 2.
    2. The registrar, dean, rector, or principal of the veterinary school must complete the appropriate parts of Section 2 and return the form directly to the Office of the Professions in an official school envelope at the address on the form.
    3. Please let us know who the contact at your school is so that we can help coordinate the process. 
  • Form 2PPE – Certification of Pre-Professional Education
    1. Select Form 2PPE – Certification of Pre-Professional Education.
    2. This form must be submitted directly by the college you attended prior to your veterinary studies. Complete the form and send it to the college(s) you attended before you began your veterinary studies. 
    3. Please let us know who the contact at your school is so that we can help coordinate the process. 

Complete Form 3 ONLY if you are licensed in another state and have not attempted the CCT examination. Applicants who have passed the NBE and CCT or have passed NAVLE and whose education has been approved by New York State are not required to file Form 3.

  • Form 3 — Verification of Out-Of-State Licensure, Registration, and/or Examination
  • Select Form 3 Verification of Out-Of-State Licensure, Registration, and/or Examination.
  • Complete Section 1 of the form before sending it to the state licensing authority. Be sure to include any fee required by the other state.
  • The appropriate official of the licensing authority must complete Section 2 and return the form directly to the Office of the Professions at the address on the form.

Complete Form 4 ONLY if you are licensed in another state and have not attempted the CCT examination. Applicants who have passed the NBE and CCT or have passed NAVLE and whose education has been approved by New York State are not required to file Form 4.

  • Form 4 – Verification of Professional Practice
  • Select Form 4 and complete Section 1.
  • A duly licensed veterinarian in good standing in the state where you are licensed must complete Section 2 and return the form directly to the Office of the Professions at the address on the form.

Relocation Process

  1. After you receive your NYS license, we will ask you to send us:
  • Your ID 
  • Limited Permit/NYS License 
  • W2 / W9 form 
  1. We will pay for your transportation to New York and provide you with an apartment close to the hospital. 
  2. We will ask you to work as a relief veterinarian for one week to see if our environment, work ethic and standard of care match what you are looking for.

If we agree to work together at the end of that week, we will start the paperwork process to welcome you as a permanent team member of Bronx Veterinary Center!
